چکیده

Objective: To determine whether the C-reactive protein (CRP) can help identify which febrile patients with sickle cell disease (SCD) require antibiotic therapy. Patients and Methods: We performed a chart review of patients with SCD (including S-S, S-C, and S-Thal) admitted to the hospital with fever [temperature >38.3C <2 years of age; >38.9C ≥2 years of age] over a thirty-month period (July 2009 – December 2011). Data extracted included temperature, age, high sensitivity CRP value, chest x-ray and culture results, and the underlying cause of fever, categorized as bacterial (antibiotics necessary) or non-bacterial (antibiotics not necessary). The Mann-Whitney U-test was then used to compare the median CRP levels of patients requiring antibiotics versus those whose illnesses did not require antibiotic treatment. Results: Fifty-four febrile patients with SCD were admitted to the hospital. Nineteen had final diagnoses that required antibiotic treatment and their median CRPs were significantly higher than the thirty-five who had diagnoses that did not meet the requirements for antibiotic treatment (100.9 mg/L vs 17.3 mg/L, p <.001). All patients who needed antibiotics had a CRP >39 mg/L. Conclusion: The current data indicate that among febrile patients with SCD, the CRP may help differentiate bacterial infections which require antibiotic treatment from other diseases for which antibiotics are not necessary. Our data suggests that there may be a level of CRP below which a bacterial etiology is unlikely.

Antioxidant Enzymes and Acute Phase Proteins Correlate with Marker of Lipid Peroxide in Adult Nigerian Sickle Cell Disease Patients

Objective(s) Sickle cell disease is a genetic disorder characterized by chronic haemolytic anaemia. Haemoglobin S containing red blood cells may be susceptible to oxidative stress due to imbalance between production of reactive oxygen species and the countering effect of the various antioxidants present in the body. Materials and Methods We evaluated some antioxidant enzymes which include gl...

Determination of Serum C, S Proteins and Factor V Leiden among Patients with Sickle Cell Disorder at Khuzestan Province, Iran

Background: Sickle cell disease occurs due to a mutation in β chains and the substitution of valine instead of glutamate in the sixth position of the ß-chain that causes polymerization and vascular blockage. The aim of this study was to compare the serum C, S proteins and factor V Leiden between sickle cell patients and the control group. Materials and Methods: In this case-control study, perf...

BENIGN EXTREME HYPERBILIRUBINEMIA IN A 9 YEAR-OLD GIRL WITH SICKLE-THALASSEMIA AND THE PROBABLE ROLE OF HBF IN PREDICTING THE OUTCOME

Hepatic dysfunction is a frequent manifestation in patients with sickle cell anemia. It is usually a multifactorial process. A rare benign form of extreme hyperbilirubinemia, presumably due to intrahepatic sickling, may be the cause. We report a 9 year old girl with sickle-thalassemia hemoglo binopathy, presenting with profound jaundice. Sickle cell disease is often mild in the Iranian popu...

C-reactive protein and other markers of inflammation in hemodialysis patients

Background: Hemodialysis patients are at greater risk of cardiovascular disease. Higher than expected cardiovascular morbidity and mortality in this population has been attributed to dislipidemia as well as inflammation. The causes of inflammation in hemodialysis patients are multifactorial. Several markers were used for the detection of inflammatory reaction in patients with chronic renal dise...
